I understand that by submitting this extension agreement, I am agreeing to make the payment by the extension date. I also understand that if I do not make the payment by the date agreed, a $30.00 default fee & a $30.00 reconnect fee will be added to this account and balance must be paid in full before my water will be reconnected. *The extension is due by the 2nd Friday of the following month*
